Predicting the need of Nursing or Care Home and Home Care

The St. Antonius hospital in Nieuwegein is an ambitious top clinical training hospital. Every year, more than 40,000 patients are admitted, of which 6,000 patients go home through Care Mediation with home care or to a nursing or care home. This outflow is very erratic with seasonal influences. At the moment, patients who go to a nursing/care home sometimes spend too long in the hospital because there is no place available yet at a VVT institution. By indicating the desired capacity in advance at the various VVT institutions, St. Antonius wants to shorten the unnecessary hospital stay. In this way we try to improve the flow (inflow-flow-outflow) in the hospital. 

Research questions to be answered: 

  • Can you predict the capacity needed in nursing/care home? 
  • How early can this be done considering a certain confidence level? 
  • Which factors in the care path of the patient lead to the need of nursing/care home?
